statement and other account information anytime by going to the NextStep Website.

2. Your current, original and signed
Beneficiary Designation Notice must be on file in Lindora’s Human
Resources Department. The designation on file in Human Resources
supersedes the information entered on the BenefitStreet website.
3. When you choose an IFA Portfolio,
we recommend you select “Yes” for Automatic Rebalance. If
you selected “No” and want automatic rebalancing, please
visit the NextStep website
to change your selection.

4/13/2009 : BenefitStreet changed to Nextstep
There has been a change in one of our 401(k) Plan service providers that we feel is a long term positive change, with only a minimal short term impact to you.
- A new company called Nextstep has replaced Benefit Street as the
401(k) Record Keeper. Nextstep is a newly formed company
that essentially transitions Benefit Street into a different, more
streamlined company with the same management and operations team.
- Participant log-in to the 401(k) website will involve automatic re-direction to the new site with an explanation about Nextstep replacing Benefit Street and that the correct site is being accessed. The actual “look and feel” of the website will be the same other than the name change.
